Information checklist
It would be helpful to make sure you have the information listed below when you talk to your Access to Work Adviser. They may not need all of it but it will help you to have it to hand. Work with your employer to get the information together.
- Your name
- The name of the organisation you work for
- The name of the person Access to Work should contact at the organisation
- The name of your line manager
- Your job title
- Your working hours
- The details of what your job entails - what do you actually do?
- How does your impairment affect your ability to do your job?
- What do you need in order to be able to do your job to the best of your ability? Be as specific as possible about what you need and about how this will help you
- Whether there’s anything else that needs to happen in the organisation to enable you to do your job effectively, for example training, support or advice for others in the organisation.
- You may need to state what your medical condition is and possibly in some detail. Whilst this can be offensive and upsetting, it’s best to know that it may come up and be prepared to deal with it in whatever way you think is appropriate.
